Job Description
Experienced Home Health Registered Nurse opportunity in Fredericksburg, VA, for a full-time 13-week contract. This role requires a compassionate and independent RN dedicated to delivering high-quality nursing care in patients homes. You will be responsible for comprehensive assessments, individualized care planning, and coordination with interdisciplinary teams to optimize patient outcomes.
Schedule & Hours:
- Monday Friday
- 8:00 AM 5:00 PM
- Field-based, conducting 35 patient visits daily with a caseload of 2532 points per week
Key Responsibilities:
- Perform detailed OASIS assessments and develop patient-centered care plans
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress and communicate changes promptly to healthcare team and physicians
- Educate patients and families on treatment plans and preventive care
- Maintain accurate documentation using the Home Care Home Base (HCHB) system
- Collaborate with therapists, social workers, and aides to ensure coordinated care
- Adhere to all regulatory and agency standards
Qualifications:
- Active RN license in good standing for practice state
- Minimum 1 year recent home health experience, preferably field-based
- Proficient in Home Care Home Base (HCHB) and OASIS documentation
- Strong clinical assessment and critical thinking abilities
- Reliable transportation with valid drivers license and insurance
- Excellent communication, organizational, and time management skills
Preferred:
- BSN degree
- Prior experience managing 2532 points weekly caseload
- Familiarity with Medicare and home health compliance standards
