A rewarding opportunity is available for an experienced and compassionate Hospice Registered Nurse to deliver exceptional end-of-life care in home health settings. This contract position is focused on providing holistic, patient-centered support to individuals and their families during their most sensitive moments. Youll play a pivotal role as part of an interdisciplinary team, ensuring comfort, dignity, and thoughtful care for hospice patients across Elgin, SC.
Schedule & Structure:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
- Expected 45 patient visits daily
- Field-based role with travel to patients homes or residential facilities (mileage reimbursement provided)
Role Responsibilities:
- Complete comprehensive nursing assessments and develop individualized care plans
- Deliver skilled nursing care, focusing on symptom management, pain control, and patient comfort
- Administer medications and treatments in accordance with physician orders and hospice policies
- Monitor patient status changes and communicate effectively with the care team
- Educate patients, families, and caregivers about disease progression, support strategies, and medication
- Collaborate closely with social workers, chaplains, aides, and volunteers
- Document care and assessments accurately in Home Care Home Base (HCHB)
- Participate in case conferences, team meetings, and quality improvement initiatives
- Adhere to all hospice protocols and regulatory requirements
Qualifications:
- Active RN license in good standing (state of practice)
- Minimum 1 year recent hospice experience
- Proven proficiency in Home Care Home Base (HCHB)
- Outstanding clinical assessment, communication, and organizational skills
- Reliable vehicle, valid drivers license, and auto insurance
- Compassionate approach and emotional resilience
- BSN and familiarity with Medicare hospice regulations preferred
