An opportunity is available for a skilled and compassionate Registered Nurse (RN) specializing in Home Health to provide exceptional patient care within their homes. This role demands a high level of clinical expertise, strong communication, and the ability to work independently while managing an active caseload. The position is based in Elberton, GA, on a contract basis.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress and communicate changes promptly with the healthcare team.
• Educate patients and families about disease processes, treatments, medications, and preventive care.
• Accurately document patient information in Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to ensure coordinated care.
• Participate in case conferences, staff meetings, and ongoing education.
• Comply with regulatory standards and agency protocols for care delivery and documentation.

Qualifications and Desired Experience:

• Active RN license in good standing within the state.
• Minimum of one year recent home health nursing experience, preferably in the field.
• Proficiency with Home Care Home Base (HCHB) electronic documentation.
• Experience utilizing OASIS assessment tools.
• Strong clinical assessment abilities and critical thinking skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to manage a flexible, field-based schedule independently.
• Excellent communication, organizational, and time management skills.
• BSN preferred.
• Experience managing a caseload of 2532 points weekly.
• Knowledge of Medicare and home health compliance standards is a plus.

Schedule:

• Full-time contract, 13 weeks in duration.
• Monday through Friday, 8:00 AM 5:00 PM.
• Caseload of approximately 35 patient visits per day.
