Deliver expert nursing care in patients homes as a skilled Home Health Registered Nurse (RN) located in Dumfries, VA. This full-time contract role offers the chance to work independently while providing personalized, patient-centered care in community settings. Ideal candidates are organized, self-motivated professionals with a strong clinical background in home health nursing and proficiency in relevant assessment tools.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, identify changes in condition, and communicate effectively with healthcare teams and physicians
- Educate patients and families regarding disease management, treatment plans, and preventive care
- Maintain accurate documentation using Home Care Home Base (HCHB) electronic system
- Collaborate with interdisciplinary teams such as physical therapists, social workers, and home health aides to ensure coordinated care
- Participate in case conferences and professional development activities
- Comply with all regulatory and agency standards for patient care and documentation
Qualifications:
- Active RN license in good standing for the state of practice
- Minimum 1 year recent home health nursing experience (field-based experience preferred)
- Proficiency with Home Care Home Base (HCHB) and experience using OASIS
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to manage a flexible schedule and work independently
- BSN degree preferred
- Familiarity with Medicare and home health compliance regulations preferred

Schedule/Hours:
- Monday through Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 35 patient visits per day (2532 points weekly)
