Experienced and compassionate Registered Nurses are needed to provide skilled care to patients in their homes, ensuring the highest standard of clinical excellence and compassionate communication. This is a full-time, 13-week contract position supporting individuals who require nursing interventions in a home health setting.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized, patient-centric care plans.
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patients progress, identifying changes in condition and promptly communicating with the interdisciplinary team.
- Educate patients and families on disease management, treatments, medication regimens, and preventive care.
- Document all patient care and communication accurately and efficiently using Home Care Home Base (HCHB).
- Coordinate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure seamless care.
- Attend case conferences, staff meetings, and participate in continuing education as needed.
- Maintain compliance with all regulatory and agency standards for documentation and patient care delivery.
Required Qualifications:
- Active RN license (in good standing) in the state of practice.
- At least 1 year of recent home health experience (field experience preferred).
- Proficiency with Home Care Home Base (HCHB) and OASIS.
- Strong clinical assessment, organizational, and communication skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Self-motivated with the ability to independently manage a field-based schedule.
Preferred:
- BSN credential.
- Experience managing 2532 points per week caseload.
- Familiarity with Medicare and home health compliance standards.
