Job Description
Deliver skilled nursing care directly to patients in their homes as a Home Health Registered Nurse (RN), providing comprehensive clinical services and patient-centered care. This full-time, 13-week contract role offers the opportunity to work independently while collaborating with an interdisciplinary healthcare team to optimize patient outcomes.
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans tailored to patient needs.
- Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate effectively with physicians and care teams.
- Educate patients and families regarding disease processes, treatments, medications, and preventive care strategies.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB).
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate care.
- Participate in case conferences, staff meetings, and continuing education as required.
- Adhere to regulatory standards and agency protocols for quality care delivery.
Qualifications and Experience:
- Active RN license in good standing for practice within the state.
- Minimum of one year recent home health nursing experience; field experience preferred.
- Proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments required.
- Strong clinical assessment and critical thinking abilities.
- Reliable transportation, valid drivers license, and auto insurance for daily travel within the assigned territory.
- Excellent organizational, communication, and time management skills.
- BSN preferred.
- Prior experience managing a caseload of 2532 points weekly is advantageous.
- Familiarity with Medicare guidelines and home health compliance standards preferred.
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM
- Expect 35 patient visits daily within field-based settings
