Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care to patients in their homes, ensuring high-quality, continuity, and patient-centered care. This full-time, 13-week contract position requires a self-motivated and organized professional comfortable working independently while maintaining clinical excellence and clear patient communication.

Working Monday through Friday from 8:00 AM to 5:00 PM, you will manage a caseload of 2532 points per week, typically involving 35 patient visits daily. The role is field-based, requiring daily travel within an assigned territory, with mileage reimbursement provided. You will utilize Home Care Home Base (HCHB) for electronic documentation and OASIS for assessments.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans
  • Provide skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
  • Monitor patient progress, identify changes, and communicate promptly with care teams and physicians
  • Educate patients and families on treatment plans, medications, and preventive care
  • Maintain accurate and timely documentation in HCHB
  • Collaborate with therapists, social workers, and aides to coordinate care
  • Participate in case conferences and continuing education
  • Comply with regulatory standards for documentation and care delivery

Qualifications:

  • Active RN license in good standing within the state of practice
  • Minimum 1 year recent home health experience; field experience preferred
  • Proficiency with Home Care Home Base (HCHB) and OASIS
  • Strong clinical assessment and critical thinking skills
  • Reliable transportation, valid drivers license, and auto insurance
  • Ability to work independently and manage a flexible, field-based schedule
  • Excellent communication, organizational, and time management skills

Preferred:

  • BSN degree
  • Experience managing 2532 points weekly
  • Knowledge of Medicare and home health compliance standards
