Providing compassionate end-of-life care in the comfort of patients homes or residential settings, this role focuses on delivering quality hospice nursing services. The position requires a dedicated Hospice Registered Nurse (RN) skilled in symptom management, pain control, and emotional support for patients and their families facing terminal illness.
Key Responsibilities:
- Conduct thorough nursing assessments and develop tailored hospice care plans.
- Deliver skilled nursing care including medication administration and symptom relief.
- Monitor patient condition changes, reporting promptly to physicians and care teams.
- Educate patients, families, and caregivers on disease progression and comfort strategies.
- Collaborate with interdisciplinary teams to address physical, emotional, and spiritual needs.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB).
- Participate in team meetings and quality improvement initiatives.
- Ensure compliance with hospice policies and regulatory requirements.
Qualifications:
- Active RN license in good standing within the state.
- Minimum of one year recent experience in hospice nursing.
- Proficiency with Home Care Home Base (HCHB) documentation system.
- Strong clinical assessment and communication skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently in a field-based role.
- Compassionate and emotionally resilient.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience in palliative or end-of-life care.
- Knowledge of Medicare hospice regulations.
Location Overview: This role is based in Cottageville, South Carolina, a community known for its friendly atmosphere and serene rural surroundings. Schedule and Contract Details:
- Full-time, 13-week contract.
- Monday to Friday, 8:00 AM to 5:00 PM, with occasional weekend or on-call duties.
- Manage 45 patient visits daily in homes, assisted living, or nursing facilities.
- Daily travel within the service area with mileage reimbursement provided.
