Providing compassionate end-of-life care in the comfort of patients homes or residential settings, this role focuses on delivering quality hospice nursing services. The position requires a dedicated Hospice Registered Nurse (RN) skilled in symptom management, pain control, and emotional support for patients and their families facing terminal illness.

Key Responsibilities:

• Conduct thorough nursing assessments and develop tailored hospice care plans.
• Deliver skilled nursing care including medication administration and symptom relief.
• Monitor patient condition changes, reporting promptly to physicians and care teams.
• Educate patients, families, and caregivers on disease progression and comfort strategies.
• Collaborate with interdisciplinary teams to address physical, emotional, and spiritual needs.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Participate in team meetings and quality improvement initiatives.
• Ensure compliance with hospice policies and regulatory requirements.

Qualifications:

• Active RN license in good standing within the state.
• Minimum of one year recent experience in hospice nursing.
• Proficiency with Home Care Home Base (HCHB) documentation system.
• Strong clinical assessment and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently in a field-based role.
• Compassionate and emotionally resilient.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience in palliative or end-of-life care.
• Knowledge of Medicare hospice regulations.

Location Overview: This role is based in Cottageville, South Carolina, a community known for its friendly atmosphere and serene rural surroundings. Schedule and Contract Details:

• Full-time, 13-week contract.
• Monday to Friday, 8:00 AM to 5:00 PM, with occasional weekend or on-call duties.
• Manage 45 patient visits daily in homes, assisted living, or nursing facilities.
• Daily travel within the service area with mileage reimbursement provided.
