An opportunity is available for a skilled Home Health Registered Nurse (RN) to provide comprehensive nursing care to patients in their own homes within the Corvallis, OR area. This contract position offers the chance to deliver patient-centered care while working independently in a field-based setting, supporting individuals through skilled assessments, care planning, and coordination with interdisciplinary healthcare teams.

Key Responsibilities:

• Perform thorough OASIS assessments and develop individualized care plans tailored to patient needs.
• Administer skilled nursing interventions such as wound care, medication management, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes in condition, and effectively communicate with physicians and the care team.
• Educate patients and families on treatment plans, medication usage, disease processes, and preventive healthcare.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to ensure integrated patient care.
• Participate in case conferences, staff meetings, and pursue continuing education.
• Adhere strictly to regulatory and agency care and documentation standards.

Qualifications & Desired Experience:

• Active RN license in good standing for the state of practice.
• Minimum of 1 year recent home health nursing experience, preferably with field visits.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical assessment and critical-thinking capabilities.
• Reliable transportation with a valid drivers license and auto insurance.
• Excellent communication, organizational, and time-management skills.
• Preferred: BSN degree, experience managing 2532 points per week, knowledge of Medicare and home health compliance.

Schedule & Contract Details:

• Full-time, 13-week contract with the possibility of extension.
• Monday through Friday, 8:00 AM 5:00 PM.
• Caseload expectation typically includes 35 patient visits daily with 2532 points per week.
• Daily travel to patient homes with mileage reimbursement provided.
