Job Description

An opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health to provide skilled nursing care in patient homes within the Commerce, GA area. This full-time contract position offers a structured Monday through Friday schedule, from 8:00 AM to 5:00 PM, with a caseload averaging 2532 points weekly (typically 35 patient visits per day). The role requires daily travel within an assigned territory, with mileage reimbursement provided.

Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify condition changes, and communicate promptly with care teams and physicians.
  • Educate patients and families regarding disease processes, treatments, medications, and preventive care.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary teams including physical therapists, occupational therapists, medical social workers, and home health aides.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Ensure compliance with regulatory and agency standards.

Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum one year of recent Home Health nursing experience, preferably field-based.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
  • Strong clinical assessment and critical thinking abilities.
  • Reliable transportation with valid drivers license and auto insurance.
  • Ability to work independently and manage a flexible schedule.
  • Excellent communication, time management, and organizational skills.

Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Experience managing caseloads of 2532 points per week.
  • Knowledge of Medicare and home health compliance standards.
