Job Description

Providing compassionate end-of-life care in patients homes or residential settings requires both clinical expertise and empathy. This opportunity invites experienced hospice registered nurses to coordinate and deliver skilled nursing support focused on comfort, dignity, and quality of life. The role involves managing approximately 45 patient visits daily, traveling within the assigned service area, and collaborating closely with an interdisciplinary team including social workers, chaplains, and certified nursing assistants.

Key Responsibilities:

  • Conduct thorough nursing assessments and develop individualized hospice care plans
  • Deliver skilled nursing care emphasizing symptom management and pain control
  • Administer medications and treatments adhering to physician orders and hospice protocols
  • Monitor patient condition changes and communicate promptly with the care team
  • Educate patients, families, and caregivers on disease progression and support strategies
  • Collaborate with the hospice interdisciplinary team to address physical, emotional, and spiritual needs
  • Complete accurate and timely documentation using Home Care Home Base (HCHB)
  • Participate in team meetings, case conferences, and quality improvement initiatives
  • Ensure compliance with hospice regulatory guidelines and policies

Qualifications:

  • Active RN license in good standing within the state of practice
  • Minimum one year of recent hospice nursing experience required
  • Proficiency with Home Care Home Base (HCHB) electronic documentation system
  • Strong clinical assessment and communication skills
  • Reliable transportation with valid drivers license and insurance
  • Ability to work independently in a field-based role
  • Compassionate and emotionally resilient approach Preferred:
  • BSN degree
  • Experience in palliative or end-of-life care settings
  • Knowledge of Medicare hospice regulations and documentation standards

Location Overview: This position is based in Charlestown, MA, a vibrant neighborhood known for its rich history and close proximity to downtown Boston. Charlestown offers charming residential areas alongside waterfront views, parks, and cultural attractions, providing a supportive community atmosphere for healthcare professionals serving local patients.

Schedule:

  • Full-time contract position for 13 weeks
  • Monday through Friday, 8:00 AM to 5:00 PM
  • Occasional weekend or on-call rotations may be required
