An exciting opportunity is available for an experienced and compassionate Registered Nurse with a background in home health care. In this field-based contract role, you will deliver skilled nursing services in patients homes, supporting their recovery and well-being while building strong relationships with patients and their families.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• 13-week full-time contract
• Caseload expectation: 2532 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Complete comprehensive patient assessments using OASIS and develop individualized care plans.
• Deliver skilled nursing care including medication administration, IV therapy, wound care, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and communicate promptly with the broader care team and physicians.
• Educate patients and families about treatment plans, disease processes, preventive care, and medications.
• Maintain timely and accurate documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members such as therapists, social workers, and aides to ensure effective, coordinated care.
• Actively participate in case conferences, staff meetings, and ongoing education.
• Remain compliant with all home health regulations and documentation standards.

Qualifications:

• Active RN license in good standing (state of practice)
• At least 1 year of recent home health nursing experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS is required
• Strong clinical judgment, communication, and self-management skills
• Reliable transportation, valid drivers license, and current auto insurance
• BSN preferred; prior experience handling a 2532 point weekly caseload and familiarity with Medicare/home health standards is a plus
