Provide expert, compassionate nursing care in patients homes as a Home Health Registered Nurse (RN) on a rewarding 13-week contract in Brunswick, ME. This role offers the opportunity to deliver patient-centered care across a diverse caseload while working independently within a supportive team environment.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans tailored to patient needs.
• Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes in condition, and communicate promptly with physicians and interdisciplinary care team members.
• Educate patients and families on disease management, medications, treatment plans, and preventive care.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to ensure coordinated care delivery.
• Participate in case conferences, staff meetings, and continuing education activities.
• Ensure adherence to regulatory requirements and agency standards in all aspects of care.

Qualifications:

• Active and current RN license in the state.
• Minimum of 1 year recent home health nursing experience, preferably field-based.
• Proficient in HCHB electronic documentation system.
• Strong clinical assessment and critical thinking skills.
• Valid drivers license, reliable transportation, and insurance.
• Ability to manage a flexible, independent schedule with excellent communication and organizational skills.

Preferred Qualifications:

• BSN degree.
• Proven ability to manage a caseload of 2532 points weekly.
• Familiarity with Medicare regulations and home health compliance standards.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM.
• Caseload averaging 35 patient visits daily.
