An opportunity is available for a skilled and dedicated Registered Nurse (RN) specializing in home health to provide exceptional care within patients residences. This role calls for a self-motivated professional with strong clinical judgment and the ability to work independently while ensuring patient-centered care.
Responsibilities include conducting comprehensive assessments using OASIS, developing individualized care plans, and delivering skilled nursing interventions such as wound care, medication management, IV therapy, and chronic disease management. Coordination with interdisciplinary teams, patient and family education, and accurate documentation through Home Care Home Base (HCHB) are essential components. Timely communication of changes in patient condition to physicians and care teams will support optimal outcomes.
Key Qualifications:
- Current RN license in good standing for the state of practice
- Minimum one year recent home health experience, preferably in the field
- Proficiency in Home Care Home Base (HCHB) documentation system
- Experience with OASIS assessments
- Strong clinical assessment and critical thinking abilities
- Reliable transportation, valid drivers license, and auto insurance
- Excellent communication, organizational, and time management skills
- Ability to manage a flexible, field-based schedule independently
Preferred Experience:
- Bachelor of Science in Nursing (BSN)
- Previous caseload management of 2532 points weekly
- Familiarity with Medicare and home health regulatory compliance
Schedule & Location:
- Monday through Friday, 8:00 AM to 5:00 PM
- Field-based, daily travel to patient homes within assigned territory (mileage reimbursement included)
- Location: Biddeford, ME
Contract Details:
- Full-time, 13-week contract with opportunities for extension or renewal
