Job Description

Join a dynamic healthcare environment as a Home Health Physical Therapist (PT) delivering skilled care directly in patient homes. This full-time, 13-week contract role focuses on improving patients mobility, strength, balance, and independence through personalized treatment plans and hands-on interventions.

Responsibilities:

  • Perform comprehensive physical therapy evaluations, including OASIS assessments for Start of Care, Recertification, and Discharge as applicable.
  • Develop and implement individualized plans of care targeting functional mobility, pain management, balance, gait training, and safety.
  • Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, transfer and fall prevention training.
  • Monitor patient progress, adjust treatment plans based on clinical findings, and communicate updates to physicians and care teams.
  • Educate patients and caregivers regarding exercise routines, safety measures, and assistive device usage.
  • Maintain accurate and timely documentation using Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary team membersincluding nurses, occupational therapists, and social workersto coordinate patient care.
  • Adhere to agency policies, Medicare regulations, and professional practice standards.

Qualifications:

  • Active Physical Therapist license in the state of practice, in good standing.
  • Minimum of 1 year recent Home Health Physical Therapy experience, preferably field-based.
  • Proficiency with OASIS assessments is required; Home Care Home Base (HCHB) experience preferred.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Strong clinical decision-making, communication, organizational, and time management skills.
  • Ability to work independently managing a flexible, field-based schedule.

Schedule/Hours:

  • MondayFriday, 8:00 AM 5:00 PM
  • Approximately 57 patient visits daily, totaling 2532 points per week.

Daily travel to patient homes within the assigned territory is required, with mileage reimbursed.
