Job Description

A dedicated Physical Therapist with experience in home health care is needed to provide compassionate, patient-focused therapy services on a 13-week contract. This role involves delivering in-home physical therapy to patients of varying needs, working collaboratively with a multidisciplinary healthcare team to foster improved mobility, function, and overall well-being.

Responsibilities:

  • Perform comprehensive initial patient assessments to determine physical condition and therapy needs.
  • Develop and implement tailored treatment plans aligned with patient goals and physician directives.
  • Provide therapeutic exercises, manual therapy, and modalities to enhance strength, range of motion, and independence.
  • Educate patients and caregivers on exercise techniques, transfers, and assistive device usage.
  • Monitor patient progress and adjust treatment plans accordingly.
  • Maintain clear, timely electronic medical records in compliance with regulatory standards.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
  • Comply with ethical, professional, and regulatory requirements.

Qualifications and Desired Experience:

  • Bachelor's or Masters degree in Physical Therapy from an accredited program.
  • Current state licensure or eligibility for licensure as a Physical Therapist.
  • Prior experience in a home health setting.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical record system.
  • Strong clinical reasoning and modification skills based on patient response.
  • Excellent communication and interpersonal abilities.
  • Compassionate, patient-centered approach with motivational skills.
  • Understanding of Medicare regulations and documentation standards for home health.
  • Valid drivers license and reliable transportation for travel to patient homes.
