An experienced and compassionate Home Health Physical Therapist (PT) is needed for a full-time, 13-week contract position based in Vienna, MO. This role involves delivering skilled physical therapy services directly in patients homes to enhance mobility, strength, balance, and overall functional independence. The ideal candidate will possess strong clinical skills, be organized, self-motivated, and comfortable working autonomously.

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, Discharge)
• Develop and implement individualized treatment plans focusing on mobility, strength, balance, gait, pain management, and safety
• Provide skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
• Monitor patient progress, adjust treatment plans as needed, and communicate changes with physicians and care teams
• Educate patients and caregivers on exercises, safety, assistive device use, and home exercise programs
• Maintain accurate documentation using Home Care Home Base (HCHB) system
• Collaborate with interdisciplinary team members including nurses, occupational therapists, and social workers
• Comply with agency policies, Medicare home health guidelines, and regulatory standards

Qualifications:

• Active Physical Therapist license in the practicing state, in good standing
• Minimum one year of recent home health physical therapy experience; field experience preferred
• Experience with OASIS assessments required; proficiency in HCHB preferred
• Strong clinical decision-making and functional assessment abilities
• Reliable transportation, valid drivers license, and auto insurance
• Excellent communication, time management, and organizational skills
• Ability to work independently in a flexible, field-based schedule

Work Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM
• Caseload averaging 2532 points weekly (~57 patient visits daily)
• Daily travel within assigned territory; mileage reimbursed
