Job Description

An opportunity is available for an experienced Home Health Physical Therapist (PT) to deliver skilled physical therapy services directly to patients in their homes. This contract role focuses on enhancing patient mobility, strength, balance, and overall functional independence through patient-centered care and effective treatment planning.

Location: Sawyerville, AL

Schedule/Hours:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations, including OASIS assessments
  • Develop and implement customized treatment plans targeting mobility, strength, gait, pain management, and safety
  • Provide skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention
  • Monitor patient progress and adjust plans accordingly; communicate effectively with physicians and care teams
  • Educate patients and caregivers on exercise protocols, safety, and assistive device use
  • Maintain timely and accurate documentation using Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary healthcare professionals
  • Comply with agency policies, Medicare home health regulations, and participate in ongoing education

Qualifications:

  • Active Physical Therapist license in good standing
  • Minimum of 1 year recent home health PT experience; field experience preferred
  • OASIS assessment experience required; proficiency in HCHB preferred
  • Strong clinical judgment and functional assessment skills
  • Reliable transportation, valid driver's license, and insurance
  • Ability to work independently in a field-based setting
  • Excellent communication and organizational skills
