Job Description

An experienced and compassionate Home Health Physical Therapist (PT) is needed to deliver skilled physical therapy services within patient homes. This role focuses on improving patients mobility, strength, balance, and overall independence through personalized care plans. Ideal candidates demonstrate self-motivation, organization, and are comfortable working independently, ensuring exceptional, patient-centered outcomes.

Key Responsibilities:

  • Conduct thorough physical therapy evaluations, including OASIS assessments for Start of Care, Recertification, and Discharge.
  • Develop and implement individualized treatment plans targeting functional mobility, strength, balance, gait, pain management, and safety.
  • Administer skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
  • Monitor patient progress, adapt treatment plans as needed, and communicate changes with physicians and care teams.
  • Educate patients and caregivers on exercises, safety, assistive device use, and home exercise programs.
  • Maintain accurate, timely documentation in Home Care Home Base (HCHB).
  • Collaborate with registered nurses, occupational therapists, speech therapists, medical social workers, and home health aides to foster coordinated care.
  • Participate in case conferences, interdisciplinary meetings, and ongoing education.
  • Adhere to agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

  • Active Physical Therapist license in good standing for the state of practice.
  • Minimum of 1 year recent Home Health Physical Therapy experience; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) is preferred.
  • Experience with OASIS assessments required.
  • Strong clinical decision-making and functional assessment skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently and manage a flexible, field-based schedule.
  • Excellent communication, time management, and organizational abilities.

Job Details:

  • Location: Saint James, MO
  • Schedule: Monday Friday, 8:00 AM 5:00 PM
  • Contract: Full-time, 13-week duration
  • Caseload: Approximately 2532 points weekly (57 patient visits per day)
  • Setting: Patient homes with daily travel (mileage reimbursement available)
