Provide expert physical therapy care as a Contract Home Health Physical Therapist in Rockwood, PA, delivering personalized treatment plans to patients in their own homes. This role offers the opportunity to make a meaningful difference by helping patients regain mobility, strength, and independence through compassionate and effective interventions.
Responsibilities:
- Perform comprehensive initial assessments to evaluate physical condition, functional abilities, and mobility challenges.
- Develop individualized treatment plans aligned with patient goals and physician directives.
- Educate patients and their families on correct exercise techniques, safe transfers, and use of assistive devices.

- Current state Physical Therapy license.
- Experience in home health and Home Care High Balance (HCHB) settings preferred.
- Valid drivers license and reliable transportation.
