Job Description

A Physical Therapist with a focus on home health is needed to provide compassionate, expert care to patients in their own homes. This full-time contract role, based in Pulaski, VA, offers an opportunity to assess patient needs, implement customized treatment plans, and collaborate closely with a multidisciplinary team to enhance patient mobility and independence over a 13-week period.

Responsibilities include:

  • Conducting thorough initial assessments of patients physical condition and mobility challenges.
  • Designing and delivering personalized therapy plans involving exercises, manual therapy, and assistive device training.
  • Educating patients and families to encourage safe and effective recovery practices.
  • Monitoring progress and adjusting treatment strategies accordingly.
  • Maintaining up-to-date electronic medical records using HomeCare HomeBase (HCHB) software.
  • Coordinating with healthcare professionals to ensure aligned patient care.
  • Complying with ethical, state, and federal guidelines relevant to home health physical therapy services.

Qualifications desired:

  • Bachelors or Masters degree in Physical Therapy from an accredited institution.
  • Current Physical Therapist license or eligibility within the state.
  • Experience in home health care settings.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records.
  • Strong clinical reasoning and patient-focused communication skills.
  • Knowledge of Medicare regulations related to home health documentation.
  • Valid drivers license with reliable transportation for home visits.
