Job Description

Provide exceptional physical therapy care in patients homes as a Home Health Physical Therapist on a rewarding 13-week contract in Milledgeville, GA. This role emphasizes patient-centered rehabilitation, collaborative care, and improving mobility and quality of life for individuals in their home environment.

Key Responsibilities:

  • Perform comprehensive initial assessments to evaluate patients physical function and mobility.
  • Develop and implement individualized treatment plans based on clinical findings and patient goals.
  • Deliver therapeutic exercises, manual therapy, and modalities to enhance strength, range of motion, and independence.
  • Educate patients and caregivers on techniques for exercises, transfers, and assistive device use.
  • Monitor patient progress, document outcomes accurately, and adjust treatment plans as needed.
  • Coordinate care through regular communication with physicians, nurses, and interdisciplinary team members.
  • Maintain compliance with ethical standards, Medicare regulations, and proper documentation requirements.

Desired Experience and Qualifications:

  • Bachelors or Masters degree in Physical Therapy from an accredited institution.
  • Current state license or eligibility for licensure as a Physical Therapist.
  • Previous experience in a home health setting preferred.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records.
  • Strong clinical assessment, critical thinking, and interpersonal skills.
  • Compassionate, motivating approach with the ability to engage patients and families.
  • Valid drivers license and reliable transportation for travel to patient homes.
  • Familiarity with Medicare regulations specific to home health services.
