Deliver skilled physical therapy care directly to patients in their homes as a Home Health Physical Therapist (PT) in Lexington, KY. This full-time, 13-week contract opportunity is ideal for a motivated, organized clinician experienced in providing patient-centered interventions to improve mobility, strength, balance, and overall function.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations including OASIS assessments for start of care, recertification, and discharge when applicable.
• Develop and implement individualized treatment plans targeting functional mobility, gait training, pain management, fall prevention, and safety.
• Provide skilled interventions such as therapeutic exercise, neuromuscular re-education, and transfer training.
• Monitor patient progress and modify plans as needed while communicating status updates to physicians and interdisciplinary team members.
• Educate patients and caregivers on exercises, safety strategies, assistive device use, and home exercise programs.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with RNs, OTs, SLPs, medical social workers, and home health aides to ensure coordinated care.
• Comply with agency policies, Medicare guidelines, and regulatory standards.

Qualifications:

• Active Physical Therapist license in good standing within the state.
• Minimum 1 year recent Home Health PT experience, preferably field-based.
• OASIS assessment experience required; proficiency in HCHB preferred.
• Valid drivers license, reliable transportation, and auto insurance.
• Excellent clinical decision-making, communication, organizational, and time management skills.
• Ability to work independently with a flexible field-based schedule.

Schedule & Location:

• Monday through Friday, 8:00 AM 5:00 PM
• Caseload averages 2532 points per week (about 57 patient visits daily).
• Work from patient homes throughout the Lexington area with mileage reimbursement.
