Job Description

Deliver personalized physical therapy services to patients in their homes through a rewarding 13-week contract opportunity in the Jacksonville, TX area. This role is ideal for a licensed Physical Therapist with home health experience who values patient-centered care and interdisciplinary collaboration.

Responsibilities:

  • Perform thorough initial assessments of patients' physical condition, functionality, and mobility challenges.
  • Design customized treatment plans aligned with patient goals and physician directives.
  • Implement therapeutic exercises, manual therapy, and modalities to enhance strength, range of motion, and independence.
  • Educate patients and their families on exercise techniques, safe transfers, and assistive device usage.
  • Monitor patient progress and adjust plans accordingly for optimal outcomes.
  • Collaborate closely with physicians, nurses, and occupational therapists to support comprehensive care.
  • Maintain timely and accurate electronic medical records via HomeCare HomeBase (HCHB) software following regulatory standards.
  • Adhere to professional and ethical standards, complying with all relevant state and federal regulations.
  • Travel to patient homes using reliable transportation.

Qualifications:

  • Bachelors or Masters degree in Physical Therapy from an accredited institution.
  • Current valid state license or eligibility for Physical Therapist licensure.
  • Prior experience providing physical therapy in home health settings.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records preferred.
  • Strong clinical reasoning and adaptability in treatment planning.
  • Excellent communication and interpersonal skills to engage patients, families, and healthcare teams.
  • Compassionate, motivating, and patient-focused.
  • Understanding of Medicare regulations and home health documentation requirements.
  • Valid drivers license with reliable transportation.
