A rewarding opportunity is available for a skilled Physical Therapist specializing in home health care to provide patient-centered therapy in a home setting on a 13-week contract. Responsibilities:

• Conduct thorough initial assessments to determine patients physical status, functional capabilities, and mobility challenges.
• Develop and implement individualized treatment plans aligning with patient goals and physician instructions.
• Deliver therapeutic exercises, manual therapy, and utilize appropriate modalities to enhance strength, flexibility, and independence.
• Educate patients and caregivers on correct exercise techniques, transfers, and assistive device usage.
• Monitor patient progress, documenting outcomes and adjusting therapies as needed.
• Collaborate regularly with physicians, nurses, and interdisciplinary teams to ensure cohesive patient care.
• Maintain precise electronic medical records in compliance with regulatory requirements.
• Uphold professional ethics and adhere to all state and federal healthcare regulations.

Qualifications:

• Bachelors or Masters degree in Physical Therapy from an accredited program.
• Current licensure or eligibility for licensure as a Physical Therapist in the state.
• Prior experience working in a home health care environment.
• Proficiency with HomeCare HomeBase (HCHB) electronic medical record software.
• Strong clinical judgment with the flexibility to tailor treatment plans effectively.
• Excellent communication skills for collaboration and patient education.
• Compassionate, motivational approach fostering patient engagement.
• Knowledge of Medicare regulations and documentation for home health.
• Valid drivers license and reliable transportation for traveling to patient homes.
