Deliver high-quality, patient-centered care in a vital field-based role as a Home Health Registered Nurse (RN). This full-time, 13-week contract opportunity offers you autonomy and the ability to make a meaningful impact in patients lives, working directly in their homes throughout your assigned territory.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete thorough OASIS assessments and develop individualized patient care plans.
- Administer skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, recognize and respond to clinical changes, and communicate proactively with the interdisciplinary team and physicians.
- Educate patients and families on disease management, medications, and preventive strategies.
- Maintain accurate, timely records in Home Care Home Base (HCHB).
- Coordinate with physical and occupational therapists, social workers, and home health aides to deliver integrated care.
- Participate in case conferences, meetings, and required continuing education.
- Travel daily to patient homes (mileage reimbursement available).
Qualifications:
- Active, unencumbered RN license in the state of practice.
- Minimum 1 year of recent home health experience (field experience strongly preferred).
- Proficiency in Home Care Home Base (HCHB) and OASIS required.
- Strong clinical assessment, organizational, and communication abilities.
- Reliable vehicle, valid drivers license, and current auto insurance.
- Able to manage an independent, flexible field-based workday.
- BSN and prior experience with a 2532 point caseload per week preferred.
- Understanding of Medicare and home health regulatory compliance is a plus.
