An experienced Home Health Physical Therapist (PT) is sought to provide skilled, patient-centered physical therapy services in a home-based setting. This full-time contract position offers an opportunity to make a meaningful impact by helping patients improve mobility, strength, balance, and overall independence. The role requires self-motivation, excellent organizational skills, and the ability to work independently within a flexible schedule.

Responsibilities:
- Conduct comprehensive physical therapy evaluations, including OASIS assessments.
- Develop and implement individualized treatment plans targeting functional mobility, gait, pain management, and fall prevention.
- Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, and transfer training.
- Monitor patient progress and communicate updates to physicians and interdisciplinary team members.
- Educate patients and caregivers on exercises, safety measures, and the use of assistive devices.
- Maintain accurate documentation using Home Care Home Base (HCHB) system.
- Collaborate closely with nurses, occupational therapists, speech-language pathologists, medical social workers, and home health aides.
- Participate in case conferences, interdisciplinary meetings, and continuing education activities.
- Adhere to agency policies, regulatory standards, and Medicare guidelines.
Qualifications:
- Active Physical Therapist license in good standing.
- Minimum of 1 year of recent Home Health Physical Therapy experience; field experience preferred.
- Experience with OASIS assessments required.
- Proficiency in Home Care Home Base (HCHB) preferred.
- Reliable transportation with valid drivers license and insurance.
- Strong clinical decision-making, communication, and organizational skills.
- Ability to work independently and manage a flexible, field-based schedule.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload averaging 2532 points per week (typically 57 patient visits daily)
- Daily travel within assigned territory (mileage reimbursed)
