An opportunity is available for a skilled Home Health Physical Therapist (PT) to deliver exceptional patient-centered care in the comfort of patients homes. This role involves providing expert physical therapy services aimed at enhancing mobility, strength, balance, and overall independence for diverse patient populations. The position requires a motivated and organized professional capable of working autonomously while maintaining strong clinical standards.

Responsibilities include performing comprehensive evaluations, developing tailored treatment plans, and collaborating effectively with interdisciplinary healthcare teams to promote positive outcomes. A key aspect is educating patients and caregivers on therapeutic exercises and safety measures. The therapist will also document care accurately using the Home Care Home Base (HCHB) system, conduct ongoing assessments with OASIS tools, and adhere to regulatory guidelines.

Key Qualifications:

• Active Physical Therapist license in good standing within the state
• Minimum of one year recent experience in Home Health Physical Therapy, preferably with field experience
• Competency with OASIS assessments is required; familiarity with HCHB preferred
• Strong clinical decision-making and functional assessment capabilities
• Valid drivers license, reliable transportation, and auto insurance
• Effective communication and organizational skills
• Ability to work independently and manage a flexible, field-based schedule

Job Details:

• Contract type: Full-time, 13-week duration
• Schedule: Monday to Friday, 8:00 AM 5:00 PM
• Caseload: Roughly 2532 points weekly, typically 57 patient visits per day
• Setting: Patient homes within a designated territory, daily travel with mileage reimbursement
