A dedicated Physical Therapist is needed to deliver compassionate, patient-centered care in a home health setting on a 13-week contract in the Fort Wayne, IN area. This role focuses on providing specialized physical therapy services in patients' homes to promote recovery, mobility, and overall well-being.

Key Responsibilities:

• Perform comprehensive initial assessments to determine patients' physical abilities, limitations, and needs.
• Develop and implement individualized treatment plans based on evaluations, patient goals, and physician instructions.
• Administer therapeutic exercises, manual therapy, and modalities aimed at increasing strength, range of motion, and functional independence.
• Educate patients and their caregivers on safe exercise techniques, transfers, and use of assistive devices.
• Monitor patient progress and adjust treatment plans accordingly.
• Collaborate with interdisciplinary healthcare teams to coordinate care and ensure effective communication.
• Maintain accurate, timely electronic medical records in compliance with regulatory standards.
• Follow ethical and professional guidelines and adhere to state and federal regulations.

Qualifications:

• Bachelors or Masters degree in Physical Therapy from an accredited institution.
• Current state licensure or eligibility for licensure as a Physical Therapist.
• Experience providing care within a home health environment.
• Proficiency with HomeCare HomeBase (HCHB) electronic medical records preferred.
• Strong clinical reasoning and critical thinking skills.
• Excellent communication skills and the ability to work collaboratively with patients, families, and healthcare professionals.
• Knowledge of Medicare regulations and home health documentation.
• Valid drivers license and reliable transportation for travel to patient homes.
