A Home Health Physical Therapist opportunity is available in Escanaba, MI, offering the chance to deliver personalized care to patients within their own homes. This full-time contract position, spanning approximately 13 weeks, involves working closely with a multidisciplinary healthcare team to enhance patient mobility and independence while supporting their recovery journey.

Responsibilities:

• Perform thorough initial assessments to evaluate patients physical function and mobility challenges.
• Develop and implement individualized treatment plans tailored to patient needs and physician directives.
• Provide therapeutic exercises, manual therapy, and modalities designed to improve strength, range of motion, and daily functioning.
• Educate patients and caregivers on exercise techniques, safe transfers, and proper use of assistive devices.
• Monitor patient progress and adjust treatment plans to achieve optimal outcomes.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
• Maintain accurate and timely electronic documentation in compliance with regulatory standards.
• Adhere to ethical and professional standards while following state and federal regulations.

Qualifications:

• Bachelors or Masters degree in Physical Therapy from an accredited program.
• Current state licensure or eligibility for licensure as a Physical Therapist.
• Prior experience in home health physical therapy.
• Familiarity with HomeCare HomeBase (HCHB) electronic medical records software.
• Strong clinical reasoning and critical thinking abilities.
• Excellent interpersonal and communication skills for effective collaboration with patients, families, and healthcare teams.
• Knowledge of Medicare regulations and home health documentation.
• Valid drivers license and reliable transportation to provide care at patient residences.
