A dedicated Physical Therapist with experience in home health care is needed to provide patient-centered therapy services in Elyria, OH. This full-time contract role, lasting approximately 13 weeks, involves delivering high-quality physical therapy within patients homes, supporting their recovery and functional independence in a comfortable environment.

Responsibilities include conducting thorough initial patient assessments, designing personalized treatment plans aligned with physician orders and patient goals, and administering therapeutic exercises, manual therapy, and modalities. The role also involves educating patients and their families on exercise techniques and assistive device use, regularly monitoring progress, and collaborating with a multidisciplinary healthcare team to ensure coordinated, effective care. Accurate documentation in electronic medical records, specifically using HomeCare HomeBase (HCHB) software, and adherence to state, federal, and Medicare guidelines are essential.

Qualifications and Desired Experience:

  • Bachelors or Masters degree in Physical Therapy from an accredited institution

  • Previous experience in a home health physical therapy setting
