Job Description
Provide skilled, patient-focused physical therapy services in the comfort of patients homes as a Home Health Physical Therapist (PT). This full-time, 13-week contract role offers the chance to make a meaningful impact by improving patients mobility, strength, balance, and independence while working with autonomy in a field-based setting.
Location: Dixon, MO

Schedule:
- Monday Friday
- 8:00 AM 5:00 PM
Responsibilities:
- Conduct comprehensive physical therapy evaluations and OASIS assessments at start of care, resumption, recertification, and discharge as needed
- Develop and implement individualized care plans targeting functional mobility, gait, pain management, safety, and strength
- Deliver skilled interventions including therapeutic exercise, neuromuscular re-education, gait and transfer training, fall prevention
- Monitor patient progress, adjust treatment plans accordingly, and maintain clear communication with physicians and interdisciplinary team members
- Educate patients and caregivers on home exercises, safety, and use of assistive devices
- Complete timely, accurate documentation using Home Care Home Base (HCHB)
- Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides
- Adhere to agency policies, regulatory standards, and Medicare home health guidelines
Qualifications:
- Active Physical Therapist license in good standing in the state of practice
- Minimum 1 year recent home health physical therapy experience with field experience preferred
- Proficiency with OASIS assessments required; experience with HCHB preferred
- Strong clinical decision-making and functional assessment skills
- Reliable transportation, valid drivers license, and auto insurance for travel to patient homes
- Ability to work independently and manage a flexible, field-based schedule
- Excellent communication, organizational, and time-management abilities
