Job Description

Provide expert care as a Physical Therapist specializing in home health, delivering personalized therapy services that enable patients to regain strength and independence within their own homes. This 13-week contract opportunity offers the chance to utilize your skills in assessing, planning, and implementing physical therapy treatments in collaboration with a multidisciplinary healthcare team.

Key Responsibilities:

  • Conduct thorough initial patient assessments covering physical condition, functional ability, and mobility challenges.
  • Develop and execute customized treatment plans aligned with patient goals and physician directives.
  • Administer therapeutic exercises, manual therapies, and modalities to enhance strength, flexibility, and daily function.
  • Educate patients and caregivers on exercise techniques, transfers, and use of assistive devices to support recovery.
  • Regularly monitor and document patient progress, adjusting care plans as needed.
  • Maintain open communication with physicians, nurses, and therapists to ensure coordinated and effective care.
  • Adhere strictly to ethical standards, professional guidelines, and regulatory compliance.
  • Utilize electronic medical records accurately and timely to document care.

Qualifications:

  • Bachelors or Masters degree in Physical Therapy from an accredited program.
  • Licensed or eligible for licensure in the state.
  • Experience in home health physical therapy practice.
  • Proficient with HomeCare HomeBase (HCHB) electronic medical records.
  • Strong clinical judgment and adaptable treatment planning skills.
  • Exceptional communication and interpersonal abilities.
  • Knowledge of Medicare regulations and documentation standards for home health.
  • Valid drivers license and dependable transportation for patient visits.
