Job Description
Deliver compassionate, patient-centered physical therapy services in home health settings, helping patients regain mobility and independence. This full-time opportunity is a 13-week contract located in Collierville, TN, ideal for a skilled Physical Therapist with home health experience.
Responsibilities:
- Perform thorough initial assessments to evaluate physical conditions, functional abilities, and mobility limitations.
- Develop customized treatment plans aligned with patient goals and physician orders.
- Provide therapeutic exercises, manual therapy, and modalities to enhance strength, range of motion, and functional independence.
- Educate patients and caregivers on exercise techniques, transfers, and assistive device use.
- Monitor progress and adjust treatment plans accordingly.
- Communicate effectively with physicians, nurses, and interdisciplinary team members to coordinate care.
- Maintain accurate and timely electronic medical records following regulatory standards.
- Uphold ethical, professional, and compliance guidelines in all care delivery.
Qualifications:
- Bachelors or Masters degree in Physical Therapy from an accredited program.
- Current state license or eligibility to practice physical therapy.
- Experience in home health physical therapy preferred.
- Proficiency with HomeCare HomeBase (HCHB) electronic medical records.
- Strong clinical reasoning and critical thinking skills.
- Excellent communication skills for collaboration and patient education.
- Compassionate approach focused on motivating and empowering patients.
- Familiarity with Medicare regulations and documentation for home health.
- Valid drivers license and dependable transportation for travel to patient homes.
