An experienced Home Health Physical Therapist (PT) is needed to provide skilled, patient-centered care to individuals in their homes. This full-time, 13-week contract role involves delivering comprehensive physical therapy services that focus on improving mobility, strength, balance, and overall functional independence. The ideal candidate is self-motivated, organized, and comfortable working independently within a dynamic healthcare environment.

Key Responsibilities:

• Conduct thorough physical therapy evaluations including OASIS assessments (Start of Care, Resumption, Recertification, Discharge).
• Develop and implement individualized treatment plans targeting functional mobility, strength, gait training, pain management, and fall prevention.
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, transfer training, and assistive device education.
• Monitor patient progress, adjust plans as needed, and communicate updates to physicians and interdisciplinary team members.
• Educate patients and caregivers on exercises, home safety, and equipment use.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate effectively with nurses, occupational therapists, speech-language pathologists, medical social workers, and home health aides.
• Participate in case conferences, interdisciplinary meetings, and ongoing education.
• Comply with all regulatory guidelines and agency policies, including Medicare home health requirements.

Qualifications:

• Active Physical Therapist license in good standing for the state of practice.
• Minimum one year of recent Home Health Physical Therapy experience, preferably with field-based care.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments required.
• Strong clinical decision-making and functional evaluation skills.
• Reliable transportation with valid drivers license and auto insurance.
• Excellent communication, time management, and organizational capabilities.
• Ability to work independently and manage a flexible, field-based schedule.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Caseload averages 2532 points weekly (approximately 57 patient visits daily)
