An experienced Home Health Physical Therapist (PT) is needed to deliver skilled, patient-centered care within a community setting. This role focuses on improving patients' mobility, strength, balance, and overall independence by providing therapy services directly in their homes. The ideal candidate brings strong clinical skills, a self-motivated approach, and the ability to manage a field-based caseload with professionalism and flexibility.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments for Start of Care, Resumption, Recertification, and Discharge.
- Develop and implement individualized plans of care targeting functional mobility, strength, gait, pain management, transfer training, and fall prevention.
- Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, and gait training.
- Monitor patient progress, update treatment plans accordingly, and communicate status changes to physicians and interdisciplinary team members.
- Educate patients and caregivers on exercises, use of assistive devices, safety measures, and home exercise programs.
- Maintain accurate and timely documentation within the Home Care Home Base (HCHB) electronic system.
- Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides to ensure coordinated patient care.
- Participate in case conferences, team meetings, and ongoing professional education.
- Comply with regulatory standards, Medicare home health guidelines, and agency policies.
Qualifications:
- Active and good-standing Physical Therapist license in the state of practice.
- Minimum one year of recent Home Health Physical Therapy experience; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
- Reliable transportation, valid drivers license, and auto insurance.
- Strong clinical judgment, communication, organizational, and time management skills.
- Ability to work independently and manage a flexible schedule in a field-based setting.
Schedule & Contract Details:
- Full-time, 13-week contract.
- Monday through Friday, 8:00 AM 5:00 PM.
- Caseload of approximately 2532 points weekly (57 patient visits per day).
- Daily travel within an assigned territory with mileage reimbursement provided.
