Responsibilities:
- Perform thorough initial assessments to determine patients physical status and mobility challenges.
- Design and implement individualized therapy plans aligned with patient goals and physician guidance.
- Instruct patients and their families on proper exercise techniques, safe transfers, and the use of assistive devices.
- Monitor patient progress and adjust treatments as needed to optimize outcomes.
Qualifications:
- Degree in Physical Therapy from an accredited institution.
- Current, valid state license to practice.
- Proven experience in home health physical therapy and home care billing (HCHB).
- Valid drivers license with reliable transportation.
