Providing skilled nursing care in home settings, this role requires an experienced and compassionate Registered Nurse to deliver patient-centered home health services. You will perform thorough assessments, develop individualized care plans, and collaborate with interdisciplinary teams to promote optimal health outcomes for patients within their homes.

Responsibilities:
- Conduct comprehensive OASIS assessments and create personalized care plans.
- Deliver nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes, and communicate promptly with physicians and care teams.
- Provide education to patients and families on disease processes and treatment plans.
- Maintain accurate and timely electronic health records using Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and aides to ensure coordinated patient care.
- Participate in case conferences, staff meetings, and ongoing education.
- Comply with all regulatory standards for documentation and patient care.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum one year recent home health experience with field experience preferred.
- Proficiency in Home Care Home Base (HCHB) and OASIS assessment tool.
- Strong clinical assessment, critical thinking, and communication skills.
- Valid drivers license, reliable transportation, and auto insurance.
- Ability to work independently in a flexible, field-based schedule.
- Bachelor of Science in Nursing (BSN) preferred.
- Experience managing caseloads of 2532 points per week.
- Knowledge of Medicare and home health compliance standards is a plus.
Schedule:
- Full-time contract (13 weeks)
- Monday to Friday, 8:00 AM 5:00 PM
- Typically 35 patient visits daily within assigned territory (mileage reimbursed)
