Job Description

Deliver expert physical therapy services to patients in their homes as a dedicated Home Health Physical Therapist on a 13-week contract. This role centers on enhancing patient mobility, strength, and independence by delivering personalized treatment plans and collaborating closely with a multidisciplinary healthcare team.

Key Responsibilities:

  • Perform detailed initial patient assessments to determine physical capabilities and limitations.
  • Create and implement individualized treatment plans aligned with patient goals and physician directives.
  • Administer therapeutic exercises, manual therapy, and modalities aimed at improving functional outcomes.
  • Educate patients and their families on exercise techniques, safe transfers, and assistive device usage.
  • Monitor patient progress regularly, adjusting treatments as needed.
  • Maintain open communication with physicians, nurses, and therapists to coordinate care.
  • Accurately document all patient interactions and treatments in compliance with regulatory standards.
  • Uphold professional ethics and adhere to state and federal home health regulations.

Desired Qualifications:

  • Bachelor's or Masters degree in Physical Therapy from an accredited program.
  • Current state Physical Therapist license or eligibility.
  • Experience in a home health care setting.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records.
  • Strong clinical judgment and ability to modify treatments based on patient progress.
  • Excellent interpersonal and communication skills.
  • Compassionate, patient-focused approach with motivation skills.
  • Knowledge of Medicare regulations for home health services.
  • Valid drivers license and reliable transportation to visit patient homes.
