Enjoy a rewarding per diem Nurse Practitioner opportunity conducting home health risk assessments across Polk County. This flexible position enables you to design your own schedule with shifts offered seven days a week. Youll make a measurable impact by performing risk assessments in the homes of Medicare Advantage plan members, supporting both annual evaluations and post-discharge care.

Schedule:

• Choose your own hours, Monday through Sunday
• Minimum weekly or bi-weekly hourly commitment
• Locations may include Salem, Dallas, Falls City, and surrounding areas

Key Responsibilities:

• Conduct comprehensive Medicare Advantage home health risk assessments
• Review patient histories, ongoing conditions, and current medications
• Offer education related to diagnoses and medication management
• Highlight gaps in care and make recommendations for follow up
• No requirement to prescribe medication or develop ongoing treatment plans

Qualifications:

• Active and unencumbered Nurse Practitioner license in Oregon
• Graduate of an accredited MSN program
• Certification from AANP or ANCC
• Current BLS/CPR certification
• At least 6 months of recent NP experience preferred
