Take on a meaningful and independent role as a Nurse Practitioner specializing in home health risk assessments throughout Pitt County. This per diem opportunity allows you to make an impact by providing high-quality evaluations in the comfort of patients homes, supporting Medicare Advantage plan members in Greenville, Winterville, Ayden, and nearby areas.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule based on your availability
• Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual risk assessments and post-discharge visits for Medicare Advantage members during home visits
• Assess patients previous and current medical history, medical conditions, and medication usage
• Educate patients about their medical conditions, prescribed medications, and preventive care
• Identify care gaps and offer actionable recommendations to address them
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification through AANP or ANCC required
• Current CPR (BLS) certification
• MSN degree earned from an accredited nursing school
• At least 6 months of recent experience as a Nurse Practitioner preferred

This role offers compensation of $80$90 per completed evaluation, as well as a mileage stipend and reimbursement for administrative time.
