Job Description
A unique opportunity is available for a Nurse Practitioner with home health expertise to provide home-based risk assessments for Medicare Advantage plan members across Hertford County, NC. This per diem position offers autonomy, flexibility, and the rewarding experience of directly impacting patient health in a home setting. Typical visits last approximately 45 minutes and may include annual health assessments or post-discharge evaluations in the communities of Ahoskie, Murfreesboro, Winton, and nearby areas.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-directed scheduling to meet your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform in-home health risk assessments for Medicare Advantage members
- Review and assess prior/current medical history, conditions, and medications
- Educate patients regarding their medical conditions and prescribed medications
- Identify gaps in care and deliver recommendations to facilitate optimal health
- No responsibility for developing treatment plans or prescribing medication
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of six months of current NP experience preferred
