Seeking an experienced Nurse Practitioner to perform home health risk assessments on a per diem basis within Nash County. This is an excellent opportunity for practitioners interested in flexible, community-based work that supports the health and well-being of Medicare Advantage members.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: create a work pattern that suits you
• Minimum weekly or bi-weekly commitment required

Responsibilities:

• Perform comprehensive annual risk assessments for Medicare Advantage members in their homes
• Review and document prior/current medical history, ongoing health conditions, and medication usage
• Deliver education and counseling on chronic illnesses and medication management during each visit
• Identify care gaps, offering recommendations and next steps to enhance member care
• Each assessment takes roughly 45 minutes; no treatment planning or medication prescribing is required

Qualifications:

• Current Nurse Practitioner license in North Carolina
• Active CPR certification (BLS)
• Board certification from AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of experience as a Nurse Practitioner is preferred

Earn between $80$90 for each evaluation. Additional mileage stipend and compensation for administrative time included.
