Deliver essential healthcare services in a rewarding, flexible per diem role focused on comprehensive home health risk assessments for Medicare Advantage plan members. In this position, you will conduct in-home visits throughout Fountain County, INincluding Attica, Covington, and Veedersburgensuring thorough evaluations and support for those most in need.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own working days and hours
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments, each typically lasting about 45 minutes
• Evaluate medical history, current health status, and medication regimens for Medicare Advantage members
• Educate patients on medical conditions, treatments, and safe medication usage
• Identify any gaps in care and recommend resolutions to enhance patient well-being
• No responsibility for creating treatment plans or prescribing medications

Desired Qualifications:

• Active Nurse Practitioner license in Indiana
• Board Certification (AANP or ANCC)
• MSN from an accredited nursing program
• Current CPR (BLS) certification
• At least 6 months of recent Nurse Practitioner experience preferred
