Deliver compassionate, high-quality care directly to patients in their homes as a nurse practitioner specializing in home health risk assessments. This per diem position gives you the flexibility to design your own schedule and make a meaningful impact throughout Oneida County, including locations such as Whitesboro, Rome, Utica, and New York Mills.

Schedule:

• Flexible shifts available any day Monday through Sunday
• Create your own schedule based on your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate medical histories, existing conditions, and medication regimens
• Educate patients about their conditions and prescribed medications
• Identify gaps in care and provide informed recommendations for follow-up
• Visits typically last about 45 minutes, encompassing annual assessments, post-discharge visits, or other risk evaluations
• No need to develop treatment plans or prescribe medicationsfocus on assessment and guidance

Qualifications:

• Current Nurse Practitioner License in NY State
• Current CPR/BLS certification
• MSN degree from an accredited nursing school
• Board Certification through AANP or ANCC
• At least 6 months of recent NP experience is preferred
