Make an impact by providing comprehensive care and insight to patients in their homes as a Nurse Practitioner. This per diem role involves performing home health risk assessments for Medicare Advantage plan members across Glynn County, GA, including Brunswick and the Golden Isles. Assessments typically take about 45 minutes and can occur as annual health reviews or post-discharge visits. You will have flexible shifts throughout the week and the freedom to create your own schedule, with a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:
- Complete annual health risk evaluations for Medicare Advantage members in their homes
- Assess patients past and current medical history, conditions, and medication usage
- Educate individuals on their health conditions and proper medication management
- Identify care gaps and provide actionable recommendations
- No requirement to create treatment plans or write prescriptions
Qualifications:
- Active Nurse Practitioner license in Georgia
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited nursing program
- Minimum of 6 months experience as a Nurse Practitioner preferred
