Make a meaningful impact in patient care by using your clinical expertise in a supportive per diem role as a Nurse Practitioner providing home health risk assessments. This position offers the opportunity to interact directly with Medicare Advantage plan members throughout Franklin County, serving locations such as Union, Stanton, Berger, and Washington. Enjoy the autonomy to set your own schedule and focus on assessments that help improve patient outcomeswithout the responsibilities of treatment planning or prescribing.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members in their homes
- Review prior and current medical history, evaluate ongoing medical conditions, and assess medication histories
- Educate patients on their health conditions and medication regimens
- Identify gaps in care and provide actionable recommendations for follow-up
- Visits typically last around 45 minutes and may include post-discharge or annual assessments
Qualifications:
- Active Nurse Practitioner license in Missouri
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months of Nurse Practitioner experience (preferred)
- MSN degree from an accredited school of nursing
This position pays $80$90 per evaluation, with additional compensation for mileage and administrative time.
