Job Description
Deliver specialized care directly to patients homes by performing comprehensive health risk assessments for Medicare Advantage plan members across Franklin County. This per diem opportunity allows you to focus on your expertise as a Nurse Practitioner in a setting that fosters one-on-one patient engagement and flexibility.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Self-schedulingchoose shifts that fit your availability
- Minimum weekly or bi-weekly hour commitment required
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments, typically lasting 45 minutes per visit
- Thoroughly evaluate a patients prior and current medical history, ongoing medical conditions, and medication usage
- Educate patients on chronic disease management and prescribed medications
- Identify and document care gaps, offering actionable recommendations for health improvement
- Focus exclusively on assessmentno medication prescribing or development of treatment plans required
Qualifications:
- Active Nurse Practitioner License issued by the state of Missouri
- Certified in Basic Life Support (BLS/CPR)
- Board Certified through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- A minimum of six months of direct Nurse Practitioner experience preferred
- Previous home health experience is a plus
This per diem position is compensated at $80$90 per evaluation, with additional mileage and paid administrative time. Youll have the ability to tailor your work schedule, providing valuable clinical assessments across multiple neighborhoods in Franklin County.
