Seize an opportunity to provide expert care in a flexible, per diem role as a Nurse Practitioner, focusing on home health risk assessments for Medicare Advantage members across Coffee County. This position allows you to work autonomously, serving individuals in Tullahoma, Manchester, Beechgrove, and neighboring communities, making a real impact from the comfort of patients homes.

Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Evaluate medical history, current health conditions, and medication regimens
- Offer education to patients regarding their health and prescribed treatments
- Identify care gaps and provide recommendations for improved outcomes
- Focus on assessment, education, and care coordinationno prescribing or treatment plan development required
Qualifications:
- Nurse Practitioner license, active and in good standing in Tennessee
- Current CPR/BLS certification
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- At least six months of recent NP experience (preferred but not required)
Compensation: Earn between $80 and $90 per evaluation, plus receive stipends for mileage and administrative tasks, rewarding you for both time and expertise.
