Job Description

There is an open Part-Time Nurse Practitioner (NP) Home Health Flexible Schedule job in Tiffin, OH. The Part-Time Nurse Practitioner (NP) will conduct home health risk assessments within Seneca County, OH. The average home health risk assessment will take approximately 45 minutes, and the nature of the visit may vary, potentially including an annual health assessment or a post-discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Tiffin, OH,and the surrounding Seneca County, OH.Seneca County, OH,includes locations like New Riegel, Attica and Bloomville.Candidates could be scheduled in any one of these locations during an individual shift.

Responsibilities:

  • Conduct annual home health risk assessments for a Medicare Advantage health plan
  • Evaluate prior and current medical history, medical conditions and medication history
  • Assess for Gaps in Care and provide recommendations to the primary care physician for gap closure
  • Provide education on medical conditions and medications

Work Hours:

  • Flexible shifts between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Preferred Experience:

  • 6 months of current NP experience is preferred

Preferred Qualifications:

  • Nurse Practitioner License in OH
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC

Education Requirements:

  • MSN degree from an accredited school of nursing
